Old Town is literally the first permanent place that the Europeans settled in California, around the 1820's. It is a small "town" of vintage buildings. There is a charming one-room school, the Wells Fargo Museum (in the Colorado House), several restored houses -- including the beautiful Casa de Bandini, the Bazaar del Mundo with it's rich, vibrant colors, Mariachis and Flamenco dancing, and the ever popular, quaint shops.
Even though the immediate park is about six blocks square, the Old Town shops and restaurants are pretty much as far as the eye can see and the feet can walk.
To the Southeast is Heritage Park with six beautiful Victorians (a Bed and Breakfast and some shops) and the first San Diego Synagogue. It's so pretty that it's definitely worth the walk.
To the East of Old Town is Presidio Park. It has the remains of the Presidio (first military fort). It was the site of the first California mission, 1769. The impressive white building on the hill is the Fr. Serra Museum.
